New Spinal Instrumentation for Thoracic and Lumbar Spine a Preliminary Clinical Report

چکیده
SUMMARY : The new system described here is a segmental square spinal instru-mentation (termed 3-S) for spinal fusion. It consists of pairs of transverse bars, hooks and longitudinal rods. The hooks are inserted bilaterally to each lateral side of the inferior articular process. The paired hook are compressed by two transverse bars and a nut. Upper and lower hooks were linked by longitudinal rods. Clinically, we have performed 15 cases from 1984, seven cases of tumor and four cases of traumatic fracture-dislocation and one of tuberculous spondylitis, spinal canal stenosis, spon-dylolisthesis and RA. The levels were thoracic spiine in eight cases, thoraco-lumbar spine in four cases, and lumbar spine in three cases. The procedure was combined with postero-lateral fusion in six cases, and anterior spinal fusion in three cases with good results except one.

Vertebral Compression Fractures after Lumbar Instrumentation
Lumbar spinal stenosis (LSS) is primarily found in an older population. This is a similar demographic group that develops both osteoporosis and vertebral compression fractures (VCF). This report reviewed a series of patients treated for VCF that had previous lumbar surgery for symptomatic spinal stenosis. Patients that only underwent laminectomy or fusion without instrumentation had a similar d...
متن کاملSpontaneous Resolution of Symptomatic Thoracic Spine Calcified Disc Herniation: A Case Report and Literature Review
Calcification of intervertebral disc (IVD) is an uncommon but well-defined clinical syndrome in children. Despite its benign nature, occasionally, calcified nucleus pulposus can herniate into the spinal canal. We report an interesting case of calcified disc herniation in the thoracic spine. Although it resulted in cord compression and syrinx formation, it well responded to conservative manageme...
متن کاملNeurophysiological assessment of thoracic and cervical pedicle screw integrity.
Transpedicular instrumentation of the thoracic and cervical spine is technically more difficult than at lumbar levels because of narrower pedicles and less sensitive intraoperative fluoroscopic assessment. Furthermore, the potential implications of a misplaced screw are greater because of the close proximity of the spinal cord and vertebral artery. Real-time confirmation of correct pedicle scre...
متن کاملIntraoperative spinal sonography in thoracic and lumbar fractures: evaluation of Harrington rod instrumentation.
Thirty-seven patients with thoracic and lumbar spine fractures were treated with Harrington rod instrumentation (HRI), and the progress and results of that surgery were monitored with intraoperative spinal sonography (IOSS). Adequate neural tissue decompression and spinal column alignment was achieved in less than one-half (14/31, 45%) of the patients in whom HRI was performed as the first step...
متن کاملMalignant Meningioma with Lumbar Spinal Metastasis (a case report)
Meningioma is considered as a benign central nervous system tumor that accounts for approximately 18% of all brain tumors. About 5% of meningioma tumors are malignant and have more aggressive clinical signs and worse prognosis compared to benign tumors. Extracranial metastasis of meningioma is rare but in the case of occurrence, it is more prevalent in lung, liver and lymph nodes. Osseous metas...
